# How to turn a Word document into an AI quiz

A Word-structure fidelity matrix and reproducible extraction test for preserving headings, lists, tables, captions, notes, text boxes, and revision state before generating source-traceable questions.

Turn a Word document into an AI quiz by preserving its structure before generating questions. Extract headings, paragraphs, lists, tables, captions, footnotes, links, text boxes, alt text, and revision state into a reviewable evidence map; then create questions only from verified units. Block publication when the answer depends on a missing header, an unresolved tracked change, decorative layout, or information the extractor silently skipped.

## Key findings, verified 5 October 2026

- Dedicated quiz tools currently advertise DOCX input, so this is a distinct source workflow rather than generic pasted text.
- Word headings and table headers carry structure that plain copied text can flatten.
- Headers, footers, text boxes, comments, footnotes, and tracked changes require an explicit inclusion policy.
- Every answer and explanation needs an evidence-unit ID tied to the saved document version.
- Extraction fidelity does not prove that the document is accurate, licensed, accessible, or suitable for assessment.

## What is a Word-to-quiz workflow?

A Word-to-quiz workflow converts a .docx file into a bounded evidence set and then uses that evidence to draft questions, answers, explanations, and feedback. It is not the same as pasting visible body text into a prompt. A Word file can encode meaning through heading levels, table headers, captions, list nesting, links, notes, alternative text, and revision metadata. If those layers are lost, a fluent question may still be unsupported.

## Reproducible Word-to-quiz test

- Save an immutable copy. Record filename, version, date, author or owner, and a checksum when available.
- Choose the authoritative revision state. Accept or reject tracked changes, resolve comments, and document exclusions.
- Inventory every document layer. Do not treat body paragraphs as the whole file.
- Assign stable evidence IDs. Use identifiers such as H2-03, P-017, T2-R4-C3, FN-04, and FIG-02.
- Reconstruct structure. Compare section order and table relationships with Word's Navigation pane and the original pages.
- Set a review state. Mark each unit verified, corrected, uncertain, excluded, or decorative.
- Generate a calibration set. Require the answer, explanation, evidence ID, and document location for five questions.
- Run item QA. Check uniqueness, distractors, accessibility, and source fidelity before generating the full set.
- Trace every final answer. Block any item whose evidence cannot be reproduced from the saved version.

## Worked example: a table loses its headers

A policy document contains a three-column table: service level, response time, and coverage window. A basic extractor returns Priority 1 | 30 | 24/7 without the headers. The generator asks, “How many hours of coverage does Priority 1 receive?” and keys 30 as the answer. The number belongs to minutes of response time, not hours of coverage.

The repair is to preserve the caption, header row, unit, and cell coordinates. The evidence record becomes T1-R2-C2 = 30 minutes and T1-R2-C3 = 24/7. If the header association cannot be recovered, exclude the row instead of guessing.

## What to do with tracked changes and comments

Tracked changes are competing propositions until someone decides which version is authoritative. Comments may contain reviewer questions, unresolved exceptions, or private working notes rather than approved source content. Before generation, produce a clean authoritative copy and a decision log. Never let an AI generator silently choose between deleted and inserted text.

## Source-bounded Word-to-quiz prompt

Using only the verified Word evidence units below, create the requested quiz. For every item return the question, correct answer, explanation, evidence-unit ID, and document location. Preserve headings, list order, table headers, units, captions, footnotes, and stated exceptions. Do not use excluded comments, unresolved revisions, decorative headers or footers, or visual information without a verified text equivalent. Flag insufficient evidence instead of filling gaps.

## Accessibility and extraction are connected

Microsoft recommends built-in heading styles for navigation and header rows for understandable tables. Those same structures improve evidence extraction. A file that communicates structure only through font size, color, tabs, or manual spacing is harder for assistive technology and easier for a generator to misread. Repairing the document structure before generation improves both accessibility and traceability, though it does not by itself establish conformance.

## Publication blockers

- The authoritative revision state is unknown.
- A question combines evidence from unrelated sections.
- A table value is detached from its row header, column header, unit, or footnote.
- An essential figure has no verified text equivalent.
- A text box, comment, footnote, or endnote is silently omitted.
- Repeated header or footer text becomes assessed content without editorial approval.
- The answer or explanation cannot be traced to the saved source version.

## What this audit does not prove

This test checks document extraction, structure preservation, and traceability. It does not prove that the Word document is correct, current, unbiased, accessible, licensed for reuse, or appropriate for the intended learners. It does not establish reliability or validity for high-stakes assessment. Sensitive documents also require an approved privacy and retention workflow before upload.

Use the [question-quality checklist](https://bestaiquizgenerator.com/blog/how-to-evaluate-ai-generated-quiz-questions) on the generated set. If the source is a PDF export rather than the original Word file, use the [PDF source-fidelity audit](https://bestaiquizgenerator.com/blog/pdf-to-ai-quiz-source-fidelity) because tagging and reading order may change during export.
